RÜPTÜRE OVER KİSTİ VE HEMOPERİTONEUM YÖNETİMİ
Özet
Rüptüre over kisti, üreme çağındaki kadınlarda akut pelvik ağrı ve spontan hemoperitoneumun en önemli jinekolojik nedenlerinden biridir. Klinik tablo, sınırlı intraperitoneal kanamadan hipovolemik şoka kadar geniş bir spektrumda seyredebilir. Bu nedenle erken tanı, doğru ayırıcı tanı ve uygun tedavi yaklaşımı hasta prognozu açısından kritik öneme sahiptir.
Bu bölümde rüptüre over kistlerinin tanımı, epidemiyolojisi, sınıflandırılması ve patofizyolojisi güncel literatür ışığında ele alınmıştır. Hemorajik korpus luteum kistleri başta olmak üzere rüptüre neden olabilen over kistleri, kanama risk faktörleri ve hemoperitoneum gelişim mekanizmaları ayrıntılı olarak açıklanmıştır. Tanısal yaklaşım kapsamında laboratuvar incelemeleri, transvajinal ultrasonografi, Doppler ultrasonografi, bilgisayarlı tomografi ve manyetik rezonans görüntülemenin yeri ile ayırıcı tanıda dikkat edilmesi gereken klinik durumlar özetlenmiştir. Ayrıca hemoperitoneumun şiddetinin değerlendirilmesinde kullanılan klinik, laboratuvar ve görüntüleme kriterleri güncel kanıtlar doğrultusunda sunulmuştur.
Bölümde konservatif ve cerrahi tedavi yaklaşımları, cerrahi endikasyonlar, fertilite koruyucu cerrahi ilkeleri, özel hasta gruplarının yönetimi, komplikasyonlar, izlem ve nüksün önlenmesine yönelik güncel öneriler ayrıntılı olarak değerlendirilmiştir. Son bölümde ise klinik uygulamayı kolaylaştırmak amacıyla kanıta dayalı bir yönetim algoritmasına yer verilmiştir.
Sonuç olarak rüptüre over kisti ve hemoperitoneum yönetiminde temel yaklaşım; hastanın hemodinamik durumunun doğru değerlendirilmesi, uygun hasta seçimi ve bireyselleştirilmiş tedavi planlamasıdır. Güncel kanıtlar, hemodinamik olarak stabil hastaların büyük bölümünde konservatif yaklaşımın güvenli ve etkili olduğunu, cerrahi tedavinin ise seçilmiş olgularda uygulanması gerektiğini göstermektedir.
