Obstetrik ve Jinekolojik Acillerde Girişimsel Radyoloji Embolizasyon ve Güncel Yaklaşımlar
Özet
Girişimsel radyoloji, obstetrik ve jinekolojik acillerin yönetiminde cerrahiye alternatif veya tamamlayıcı, minimal invaziv bir seçenek olarak giderek daha fazla yer bulmaktadır. Bu bölümde, postpartum hemoraji, plasenta akreata spektrumu, sekonder postpartum hemoraji (uterin arter pseudoanevrizması, arteriyovenöz malformasyon, hipervasküler retansiyone gebelik ürünleri), servikal ve sezaryen skar gebeliği, gestasyonel trofoblastik hastalık gibi obstetrik acillerin yanı sıra jinekolojik malignitelere bağlı kanamalar, postoperatif ve iyatrojenik kanamalar, myom ilişkili acil kanamalar ve pelvik travma gibi jinekolojik aciller ele alınmaktadır. Girişimsel radyolojik yaklaşımların temel avantajları arasında fertilite ve uterus koruması, düşük komplikasyon oranları ve kısa hastanede kalış süresi yer almaktadır. Klinisyenin karar sürecinde asıl belirleyici faktörün altta yatan patolojinin türünden çok hastanın hemodinamik stabilitesi olduğu vurgulanmakta; erken ve zamanında yapılan girişimsel radyoloji konsültasyonunun tedavi başarısını artırdığı ve gereksiz histerektomilerin önüne geçebileceği belirtilmektedir. Bölüm, güncel kılavuzlar (FIGO 2022, JSIR) ve yeni tekniklere (REBOA) de değinerek, klinisyene günlük pratikte pratik bir referans çerçevesi sunmayı amaçlamaktadır.
