KAHVERENGİ VE BEJ YAĞ DOKUSUNUN METABOLİK ROLÜ: MOLEKÜLER DÜZENLEME, ORGANLAR ARASI İLETİŞİM VE TEDAVİ POTANSİYELİ
Özet
Adipoz doku, yalnızca enerjinin trigliseritler biçiminde depolandığı pasif bir rezerv değil; enerji homeostazı, termogenez, insülin duyarlılığı, inflamasyon ve organlar arası iletişim üzerinde etkili, heterojen bir metabolik organdır. Beyaz yağ dokusu temel olarak enerji depolama ve endokrin işlevleriyle öne çıkarken, kahverengi ve bej adipositler mitokondri yoğunlukları ve ayrıştırıcı protein 1 aracılı termojenik kapasiteleriyle enerji harcamasına katkı sağlamaktadır. Bu bölümde, 2020 ve sonrasında yayımlanan açık erişimli çalışmalar temel alınarak beyaz, kahverengi ve bej yağ dokularının yapısal ve işlevsel özellikleri; obezite sırasında gelişen adipoz doku yeniden şekillenmesi; beyazlaşma ve bejleşme süreçleri ile adaptif termogenezin moleküler mekanizmaları ele alınmıştır. Ayrıca PGC-1α, PRDM16, PPAR’lar, AMPK ve SIRT1 gibi düzenleyici moleküllerin yanı sıra batokinler, lipokinler, metabolitler ve hücre dışı veziküller aracılığıyla gerçekleşen organlar arası iletişim değerlendirilmiştir. Soğuk maruziyeti, adrenerjik uyarı, farmakolojik ajanlar ve mikrobiyota temelli yaklaşımların kahverengi ve bej yağ dokusunu hedefleme potansiyeli güncel deneysel ve klinik bulgular doğrultusunda tartışılmıştır. İnsan çalışmalarında kullanılan kahverengi yağ dokusu ölçüm yöntemlerinin farklı biyolojik özellikleri yansıtması, mevcut bulguların karşılaştırılmasını ve klinik sonuçlara aktarılmasını sınırlandırmaktadır. Bu nedenle kahverengi yağ dokusu aktivasyonu ve beyaz yağ dokusunun bejleşmesi, obezite ve kardiyometabolik hastalıklar açısından umut verici olmakla birlikte, güvenlilik ve uzun dönem etkinlik yönünden daha kapsamlı insan çalışmalarına gereksinim bulunmaktadır.
