Özet

İrritabl bağırsak sendromu (İBS) kadınlarda daha sık görülen, patolojisi tam olarak bilinmeyen bir hastalıktır. İBS’nin dışkılamayla ilişkili değişiklikler ve karın ağrısı ile karakterize olduğu, beyin-bağırsak etkileşimi bozukluğu sonucu ortaya çıktığı düşünülmektedir. İBS dünya çapında her 10 kişiden birini etkilemektedir. Hastalık; stres, enfeksiyon geçirme, bağışıklık sisteminin bozulması, visseral aşırı duyarlılık veya genetik bir defektten kaynaklanabilmektedir. İBS, semptomlarına göre alt tiplere ayrılmaktadır: Kabızlığın baskın olduğu (İBS-C), diyarenin baskın olduğu (İBS-D) ve her ikisinin birden görülebildiği karma (İBS-M) alt tipleri mevcuttur. Hastalarda tespit edilen bu baskın bağırsak alışkanlıklarına göre uygun tedavi yöntemi belirlenmelidir.

Hastalığın yönetimi için çeşitli yöntemler uygulansa da ilk amaç tanı sürecinin etkinliğini ve doğruluğunu artırmaktır. Günümüzde İBS’nin tanısı için spesifik bir biyobelirteç bulunmamaktadır. İBS şüphesi olan hastalara tam kan sayımı, IgA, CRP, fekal kalprotektin, TSH, sitokin ve kemokin ölçümleri yapılmaktadır. Bu biyolojik parametrelere ek olarak, tanı sürecini kolaylaştıracak spesifik biyobelirteçlerin keşfi büyük önem taşımaktadır.
