Özet

İlk trimester kanamaları, gebeliğin erken döneminde en sık karşılaşılan obstetrik acillerden biri olup, hafif ve kendini sınırlayan klinik tablolardan maternal yaşamı tehdit eden ciddi durumlara kadar geniş bir spektrum göstermektedir. Bu bölümde ilk trimester kanamalarına güncel ve sistematik yaklaşım ele alınmıştır. İlk olarak epidemiyoloji ve patofizyolojik mekanizmalar özetlenmiş, ardından anamnez, fizik muayene, vital bulgular ve laboratuvar değerlendirmesinin önemi vurgulanmıştır. Serum β-hCG ölçümleri ve transvajinal ultrasonografinin tanısal süreçteki kritik rolü ayrıntılı olarak incelenmiştir. Spontan abortus tipleri; abortus imminens, inkomplet abortus, komplet abortus, missed abortus, septik abortus ve tekrarlayan gebelik kaybı başlıkları altında değerlendirilmiş, ayırıcı tanıda dikkat edilmesi gereken klinik özellikler özetlenmiştir. Ayrıca ilk trimester kanamalarının önemli nedenlerinden olan ektopik gebelik ve mol gebelik güncel tanı ve tedavi yaklaşımları ışığında ele alınmıştır. Bölümün son kısmında expectan, medikal ve cerrahi tedavi seçenekleri ile takip ve danışmanlık ilkeleri sunulmuştur. Erken tanı, doğru sınıflandırma ve uygun tedavi yaklaşımı sayesinde maternal morbidite ve mortalitenin önemli ölçüde azaltılabileceği vurgulanmıştır.
