ARE PROBIOTICS AND PREBIOTICS EFFECTIVE ADJUNCTIVE THERAPIES IN PARKINSON’S DISEASE?

Özet

Parkinson's disease (PD) is a progressive neurodegenerative condition, pathologically distinguished by the formation of intraneuronal composed of α-synuclein aggregates, which contribute to disease pathology and the development of both motor and non-motor clinical features. Regarding those diagnosed with PD, α-synuclein aggregation, enhanced intestinal permeability, bacterial translocation, and increased oxidative stress levels have been observed. PD associated with gut dysbiosis may be managed through dietary interventions, particularly with the use of probiotic and prebiotic supplementation. Therapeutic interventions such as probiotics can positively modulate the gut environment and, through their antioxidant properties, help limit excessive free radical production, thereby potentially slowing disease progression and reducing disease risk. Although several studies have investigated probiotic supplementation, research on prebiotics and synbiotics remains very limited.Recent findings have demonstrated that probiotic enrichment exerts promising beneficial effects in various human and animal models of disease and yields positive outcomes in improving disease symptoms. Studies suggest that probiotics may be particularly effective in alleviating Parkinson’s disease–related gastrointestinal dysfunctions and may also have potential value in improving motor symptoms, possibly through reducing inflammatory responses and fortifying neuronal function along the gut-brain axis. However, despite the fact that high-quality studies generally report improvements in disease symptoms, current evidence remains insufficient to support the routine use of prebiotics and probiotics in clinical practice for patients with PD. Further larger sample sizes, and well-designed randomized controlled trials are required. This section will comprehensively address the gut microbiota changes associated with PD, the potential mechanisms of action of prebiotics, probiotics, and synbiotics, and the current clinical evidence supporting the use of these approaches.
